How To Apply

Applications are accepted and reviewed on a rolling basis with at least two disbursements per year as recommended by the allocations committee and approved by the board of directors.

Applicants must be a non-profit or educational entity and must supply a current W-9 tax identification number.

  • A letter outlining the following should be submitted to the Johnstown Hospital Foundation when requesting funding:
    • Name of Organization
    • Tax ID Number
    • Amount of Request
    • How funding will be used – detailed explanation of the use, estimates, if appropriate for purchases or renovations, and timeline for when the project will begin and be completed.
    • Of the total project, how much is being requested of the Johnstown Hospital Foundation?
    • Are there other donors funding this project? If so, how much and by whom?
    • Is this funding part of a challenge to raise a specific amount?
    • Will the funds requested from the Johnstown Hospital Foundation complete the challenge? If not, will the challenge need to be met before the project begins?
    • Any other information the board should know/consider?
